Modification of a task - Teacher only
If a teacher needs to change an assessment task they will complete this form and give to the Assessment Committee for approval.

Do you feel you qualify for Special Consideration?
Have you been unwell?
Do you have a family commitment?
Did unforseen circumstances impact upon your performance in a task?
If so, you may submit a Special Consideration (Extension, Illness/Misadventure, Appeal) form to the assessment committee to state your case.
To submit, email to canowindra-h.school@det.nsw.edu.au to give to the Assessment Coordinator within the required timeframe.
